ABSTRACT

EL objective of this study is to analyzethe relationship between the indicators of food security, undernourishment and the cost of the food basket in thecontext of public policies on food in Venezuela, during theperiod between 2017 and 2022. For this, an exhaustive analysisof the available data on food security in Venezuela wascarried out, including information on undernourishment, andthe cost of the food basket. Likewise, the public policiesimplemented in the country regarding food during the studyperiod were analyzed, in order to understand their impacton food security indicators. None of the associations studiedturned out to be statistically significant (p>0.05), therefore, although theoretically there is a relationship between theseindicators in the period studied, the multidimensional natureprevails and makes comparability complex. Public policies that require improvements or adjustments to protect Venezuelanfood security were identified. These results obtained may beuseful for those responsible for decision-making in the country,as well as for researchers and professionals interested in thesubject of food security and nutrition.

ABSTRACT

The Normative Food Basket (NFB) represents astrategic planning and monitoring instrument, which impactsthe economic sphere (setting of the minimum wage (MW) andthe relative poverty threshold), food security and public health. The objective was to describe the historical evolution of the NFB in Venezuela, contrasting its economic valuation with respect to the MW during the period 1990 2023. Type of study: Descriptive. The NFB of the National Institute of Statistics/National Institute of Nutrition (NIS/NIN) was used and itscomparison with the basket of the Center for Documentationand Social Analysis of the Venezuelan Federation of Teachers (CENDAS FVM). The monthly values of the NFB and theMW were recalculated into dollars (US$), according to theofficial exchange rate. No evaluation of the internal structurewas included, nor its expressions in terms of energy and nutrientcontribution. Results: From 1990 to 2015, between 1.0 and1.8 MW and between 0.6 and 1.7 Minimum Legal Income(MLI) were required to access the NFB. By 2023, up to 78.3MW and 51.6 MLI were required. The average value of the CENDAS-FVM basket was higher than the value of the NFBNIS/NIN, in a proportion of 1.7: 1. Conclusions: As a statistical operation, the NFB was sensitive in identifying changes andtrends in its estimate economic, in the Venezuelan inflationaryenvironment. The use of its results is subject to a certain degree ofpolitical discretion. The cost of CAN expresses a contraction inthe purchasing power of Venezuelan households with potentialimpacts on nutrition and physical/mental health in the shortand long term.

ABSTRACT

Carcinoma of unknown primary (CUP) is a kind of metastatic tumor whose primary origin cannot be identified after adequate examination and evaluation. The main treatment modality of CUP is empiric chemotherapy, and the median overall survival time is less than 1 year. Compared with immunohistochemistry, novel method based on gene expression profiling have improved the sensitivity and specificity of CUP detection, but its guiding value for treatment is still controversial. The approval of immune checkpoint inhibitors and pan-cancer antitumor agents has improved the prognosis of patients with CUP, and targeted therapy and immunotherapy based on specific molecular characteristics are the main directions of future research. Given the high heterogeneity and unique clinicopathological characteristics of CUP, "basket trial" is more suitable for clinical trial design in CUP.

ABSTRACT

Objective:To evaluate a newly designed basket fit for digital cholangioscope in non-radiation endoscopic lithotomy for common bile duct stones.Methods:Seventy-eight patients who underwent non-radiation endoscopic removal of common bile duct stones at Zhongda Hospital Southeast University between February and October 2021 were enrolled in the randomized controlled trial, and were randomly assigned into the new basket group ( n=35) and the conventional basket group ( n=43) by drawing lots. Procedures of cholangioscope-based biliary exploration and stone removal by baskets, volume of water irrigation, time lengths of biliary exploration, stone removal and whole endoscopic procedure, and adverse events were compared. Results:Compared with the conventional basket group, the new basket group needed less biliary explorations (1.08±0.28 VS 2.30±0.51, t=-12.535, P<0.001), smaller volume of water irrigation (46.14±11.89 mL VS 78.62±10.09 mL, t=-13.052, P<0.001), and shorter time of biliary exploration and stone removal (9.69±2.97 min VS 12.67±2.51 min, t=-4.815, P<0.001) and whole endoscopic procedure (30.17±7.19 min VS 33.44±6.69 min, t=-2.076, P=0.041). The incidence of adverse events was not significantly different between the two groups [5.7% (2/35) VS 11.6% (5/43), χ2=0.826, P=0.363]. Conclusion:This newly designed basket can facilitate digital cholangioscope-guided non-radiation endoscopic removal of common bile duct stones with the advantages of shorter operation time, less exploration time of choledochoscopy and smaller volume of water irrigation.

ABSTRACT

ABSTRACT Objectives. To evaluate and compare the macronutrient distribution of the food baskets delivered by Peruvian municipalities during the COVID-19 pandemic according to the geographic domain and assigned budget level. Materials and methods. Secondary analysis of the database "Consultation of Acquisition and distribution of basic necessities of the basic family basket" of the General Comptroller of the Republic. Stratified probability sampling was carried out. The caloric intake distribution was calculated according to macronutrients and compared with the Acceptable Intervals of Macronutrient Distribution (IADM) of the Nutrition Institute of Central America and Panama (INCAP) and the National Institute of Civil Defense (INDECI). Results. At the national level, the median caloric intake of proteins was 7.7%; for carbohydrates it was 62.5%; and for fats it was 28.1%. The proportion of municipalities with protein deficit was 84%; Municipalities with excess carbohydrates ranged from 16.5% (according to INCAP) to 35.9% (according to INDECI), and with excess fat, it was between 61.6% (according to INCAP) and 20.2% (according to INDECI). According to INDECI, nationally only 9.2% of municipalities delivered baskets with an adequate distribution of macronutrients; Metropolitan Lima stands out with the highest proportions of adequate baskets, while in the Jungle region this percentage was less than 5%. Conclusions. Most of the baskets delivered did not have adequate macronutrient distribution. Especially the baskets of municipalities outside of Metropolitan Lima or those that had smaller budget. Carbohydrates and fats were the nutrients that were included in excess, while proteins were deficient.

ABSTRACT

This paper introduced the basic definition, application scope, advantages and challenges of the master protocol, basket design, umbrella design and platform trial, and put forward the idea of using master protocol, basket design and umbrella design in Chinese medicine(CM) by considering the characteristics of CM and research experiences. The author pointed out that master protocol, basket design and umbrella design, as a high-efficiency research and design strategy, can be used in the clinical research on the treatment of the same disease with different therapies, the treatment of different diseases with the same therapy and the combination of diseases and CM syndromes. In particular, the exploration from the classification of CM syndromes can supplement the gaps in the cli-nical research on CM syndromes. In the application of such designs, it is also necessary to pay attention to their potential challenges and develop reasonable and feasible plans on research implementation, management and statistical analysis in advance to meet these challenges.

ABSTRACT

Abstract: Introduction: The Basic Food Basket (CBA) consists of a list of foods and beverages that represent current food habits in Costa Rica. Criteria for including an item in the CBA are universality, energy and protein contribution and percentage of total expenditure on food. The objective of this study is to demonstrate that the inclusion of nutritional criteria can improve the structure of the current CBA, without increasing cost and contribute to a greater congruence with the human right to food. Methodology: The quantities of foods included in the current urban CBA were compared with the Food Guideline recommendations and the nutritional content of foods in the CBA was compared with the daily nutrient recommendations. Suggested modifications were made so that the CBA has a better nutritional profile and its cost calculated to see the impact at the economic level. A recreation of the amounts of CBA food for one day was performed. Results and discussion: 21 additional foods were incorporated into the suggested CBA, six were eliminated, two foods were increased in quantity and in one quantity was decreased. It is possible to increase the contribution of dietary fiber, calcium, magnesium, potassium, zinc, vitamins C, E and K and reduce the contribution of total sugar, added sugar, sodium and cholesterol without increasing cost; in fact the cost of the modified CBA is slightly reduced. Conclusions: It is possible to include additional nutritional criteria such as micronutrient content in the formulation of the CBA without increasing cost. The structure of the CBA should be reviewed to offer a more adequate, balanced, and varied diet that responds to the current nutritional needs of the population in Costa Rica.

ABSTRACT

Abstract The scope of this study was to assess the evolution of costs for purchasing the basic food basket associated with the purchasing power of the minimum wage, besides measuring the availability per capita in Brazilian households of the items comprised in the food basket. Additionally, adequacy of the nutritional composition and diet quality based on daily consumption of food basket items was evaluated. The results obtained indicate the existence of periods of price increase for the acquisition of the food basket items, as well as the inability of the minimum wage to meet the cost of such food supplies. It was verified that the diets based on the consumption of food basket items have a higher calorie and sodium content than the recommendations for adults, as well as insufficient amounts of calcium, potassium, and vitamin A. The evaluation of the quality of the diets indicated low amounts of fruit, vegetables, cereals in the diet consisting of the items of the food baskets, with excess calories from fats and added sugar being verified. In view of such evidence, a review of the composition of the food baskets is recommended in order to meet basic nutritional requirements in food for the Brazilian population.

ABSTRACT

Introduction: Argentinian population exhibits a high prevalence of celiac disease (1/100) and feeding is the only treatment; therefore, the cost of diet is a key factor for adherence. Objectives: to calculate the differential cost of the diet of an adult celiac woman vs. non-celiac one, based on the Basic Food Basket (BFB) and on the dietary pattern proposed by the Dietary Guidelines for the Argentine Population (GAPA). Estimate the proportion that represents based on the Minimum Living Wage (MLW) in both cases. Materials and method: tracers extracted from the digital platform of an hypermarket were taken during September and December 2018 to calculate the cost of the diet, considering the lowest-priced foods. Results: the cost of food proposed by GAPA showed a difference of 55.27% higher for the celiac population ($ 1410.08 more) in September and of 48.60% ($ 1537.63 more) in December. The monthly cost represented an increase of 13.18% for the MLW in September and 13.61% in December for the celiac population. Taking the BFB, the differential cost of the monthly diet was higher (89.27% more expensive, equivalent to 1182.76 pesos more) in September, and 46.81%, equivalent to $ 886.10, in December. It represented an increase of 11.06% for the MLW in September and of 7.84% in December. Conclusions: the food for a celiac adult woman is higher than for a nonceliac adult woman in CABA. This is one of the factors that contributes to non-adherence to the treatment of the disease; food being the only strategy to treat it(AU).

ABSTRACT

Introduction: ensuring the population's accessibility to a healthy diet requires addressing the conformation and monitoring of the value of a healthy food basket and the differential prices of foods following nutritional quality. Objectives: to design a healthy food basket (HFB) consistent with the recommendations given by Argentina's dietary guidelines (GAPA). The specific objectives were to define a nutritional quality index for foods and beverages, the composition of HFB by food group, assess values, and compare the prices and nutritional quality of foods available in local markets. Methods: the methodological framework was based on GAPA and the concept of nutritional density. To define the Nutritional Density Index (NDI) of food, 9 essential nutrients (proteins, fiber, calcium, iron, zinc, potassium, Vitamins A, C and B9), 3 critical nutrients (sugars, sodium and saturated fatty acids), the starch content, and the recommended intakes of the Institute of Medicine (IOM) were considered. Specifications were prepared on quantities, variety and qualities of food for the HFB, according to the food group. The prices were registered in shops in the City Centre (CABA) and neighboring municipalities of Buenos Aires. Results: when applying the NDI to a set of 320 foods and beverages consumed by the Argentine population, they were classified into three tertiles of quality. The resulting order is consistent with the recommendations of GAPA. The value of the HFB was $ 3759.- monthly (March 2018) for the reference unit of 2000 kcal. When comparing nutrient densities and food prices, starchy vegetables, eggs and legumes are the categories with the best nutritional quality / price ratio. The foods of better nutritional quality have a price of $ 14.- per 100 kcal and those of minimum quality $ 4.5.- Discussion: the HFB and NDI are tools to guide and evaluate policies that promote a better diet quality.

ABSTRACT

Gamma band oscillation (GBO) and sensory gating (SG) are associated with many cognitive functions. Ketamine induces deficits of GBO and SG in the prefrontal cortex (PFC). However, the time-courses of the effects of different doses of ketamine on GBO power and SG are poorly understood. Studies have indicated that GBO power and SG have a common substrate for their generation and abnormalities. In this study, we found that (1) ketamine administration increased GBO power in the PFC in rats differently in the low- and high-dose groups; (2) auditory SG was significantly lower than baseline in the 30 mg/kg and 60 mg/kg groups, but not in the 15 mg/kg and 120 mg/kg groups; and (3) changes in SG and basal GBO power were significantly correlated in awake rats. ABSTRACT

ABSTRACT

Objective To investigate the application effect of stone basket in ureteroscopic holmium laser lithotripsy for treatment of upper ureteral calculi. Methods The clinical data of 96 patients with upper ureteral calculi were retrospectively analyzed, all patients underwent ureteroscopic holmium laser lithotripsy. Forty-eight cases used the stone basket in the operation process (observation group), and 48 cases did not use the stone basket in the operation process (control group). The operation time, length of stay, success rate of lithotripsy, stone residual rate and incidence of postoperative complication were compared between 2 groups. Results The patients of 2 groups successfully completed surgery. There were no statistical differences in operation time, length of stay and incidence of postoperative complication between 2 groups (P>0.05). The success rate of lithotripsy in observation group was significantly higher than that in control group: 97.92% (47/48) vs. 75.00% (36/48), and the stone residual rate was significantly lower than that in control group:4.17%(2/48) vs. 18.75%(9/48), and there were statistical differences (P<0.05). Conclusions The stone basket in ureteroscopic holmium laser lithotripsy for treatment of upper ureteral calculi can thoroughly remove stones. It reduces the incidence of residual stones, does not affect the safety of the operation, and has good clinical value.

ABSTRACT

OBJECTIVE: to determine factors related to the composition of the nutritious food basket of families living in the city of La Paz. MATERIAL AND METHOD: this is a transversal analytical study of 422 families in the city of La Paz, of the 7 Macrodistricts: Cotahuma, Max Paredes, Periférica, Centro, San Antonio, Sur and Mallasa. A survey with socioeconomic variables (Graffar-Mendez Castellanos Method) was applied. Food habits and consumption were registered (in order to determine the quantity, quality, harmony and adaptation of the consumed diet), the physical and economic access to food (in order to determine the percent of the family's income allotted to the purchase of food). RESULTS: It can be asserted that the composition of the family's cost of the basic nutritious food basket, in quantity and quality keeps a relationship with socioeconomic factors, food habits and physical access to food with less than 5 percent significance. (p<0,05). Urban families of La Paz made up of 3,5 to 4,4 members require at least 2.5 to 3.5 minimum wages only in order to cover food expenditures. The family income percent allotted to food is around 40%. Families with lesser incomes allot a higher income percent to food and families with higher incomes allot a lesser income percent to food. CONCLUSIONS: purchase capacity of families in order to reach an adequate basic nutritious food basket is limited and it is mainly related to the socioeconomic level, the family's physical and economic access. Although minimum wages have been increased over the pastyears, there are various already mentioned factors determining the composition of a family's basic nutritious food basket.

ABSTRACT

The purpose of this study was to describe anthropometric characteristics and body composition of elite soccer and basketball players as well as to make comparisons between them. Seventy-one males were enrolled in the study, divided into three groups: twenty-six soccer players, fourteen basketball players and thirty-one healthy sedentary subjects. All subjects were assessed for the anthropometric measures required for the calculation of body composition variables, using the standardized procedures recommended by established literature. Data was analyzed using SPSS and the descriptive statistics were expressed as a mean (SD) for each variable, while the ANOVA and LSD Post Hoc tests were carried out to detect the effects of each type of sport. The results showed there was no significant difference in body mass index among the groups, while a significant difference was found for body height and body weight as well as for all three of the body contents measured (muscle, bone and fat) among the groups. Basketball players were significantly taller and heavier than soccer players and the subjects of the control group, while there was also a significant difference between the weight of soccer players and the subjects of the control group, who were significantly heavier. The muscle and bone contents in the bodies of the subjects in the control group were significantly lower than that of all other subjects while there was no difference between soccer and basketball players. Lastly, the fat content in the body of the subjects in the control group was significantly higher than that of all of the other subjects, while there was no difference between soccer and basketball players...

ABSTRACT

Ensuring access in quantity, quality and regularity to foods is a constitutional and human right. As a result, public food safety programs seek to focus their efforts on the most socially vulnerable populations. The main objective of this work was to investigate the energetic contribution and macronutrients composition for the diet food basket offered by Programa Cidadania Alimentar (PCA) from the city of Canoas to the nutritional state of diabetic elders. A descriptive study was conducted, including 163 diabetic elderly of both genders enrolled in PCA, who received the benefit of a diet package food. It was perceived that 76.1% of the enrolled participants are female, 68.1% showed body mass index (BMI) classification of >27kg/m², indicating overweight. The diet food basket composition supplied by the PCA includes the average of 924.4 kcal/day, distributed in 54.2% of carbohydrates, 13.4% of proteins and 32.5% of fats. The results showed good energetic adequacy of a diet food basket when the estimate energy requirements (ERR) for elders is taken into account. However it is suggested a program reevaluation in order to promote health acts considering BMI found in this population.
